Laguna Health, the AI-powered contextual care management platform, has announced the initial close of its Series A funding round, securing $15 million in investment.

The round was co-led by SemperVirens and HC9 Ventures, bringing Laguna’s total funding to over $21 million.

The company plans to utilize the funds to enhance its AI capabilities in the context of care management and expand its go-to-market strategy.

Currently, around 35 million Americans are hospitalized annually, and a majority of them are discharged to self-care at home. However, patients often receive minimal ongoing clinical oversight and rely on paper instructions for their recovery.

Laguna Health aims to address this issue by integrating medical care plans with social, emotional, and cultural factors using its AI engine. By considering patients’ unique life contexts, the platform assists care managers in providing personalized support to overcome recovery barriers.




The lack of attention to individual life circumstances during care transitions can be costly, with non-medical errors being seven times more expensive than medical errors. Factors like arthritis, poor vision, caregiver responsibilities, or work obligations can hinder patients’ adherence to treatment plans.

Laguna’s AI engine employs natural language processing (NLP) to identify these contextual factors and offer targeted interventions such as personalized content and virtual support from care managers.

Employers and health plans are increasingly seeking solutions that improve patient outcomes and provide a positive return on investment. Laguna Health aims to address these needs by offering its contextual care management platform through health plans, requiring no changes in employer plan design.
